Rolling on half a dozen 35" tires is a good part of your problem. I've got 106k miles on my dually and ride on 285/70R17's and average 13 in the city. Figure 15 on the highway, when I'm not in a rush, and I average 10 mpg when towing our 27' TT or 12k lbs of horses and gear.
Also slowing down and pulling in 5th gear helps a ton.

My sweet spot is 5th gear and 67 mph. How are your transmission temps?
 
I will have to look on my next trip and report back. Heading back to Glamis next weekend . When pulling do you ever go into 6th gear?
If your tires were stock, I don't see a problem with pulling in 6th on flat land and not at full gross weight. You do not want the tranny hunting between 5th and 6th. The constant shifting is what wears out the clutch packs. Transengineer is the one to ask if it's advisable to pull in 6th with your setup.
