04.5 went to the high output version.
False info, the 03 cali was a 235hp edition truck and anything other than that 03-04 that had the 250hp motor was the SO, the 305hp 03-04 was the HO motor in the series, every motor in the 04.5+ series went to 325hp there is no HO motor for the 04.5+ series although they did change the TQ ratings on the 05's+ from 600ft lbs to 610 ft lbs (i believe thats when they changed it), other than that they are all equal. The 2003-2004 Trucks have 2 injection events and the 2004.5+ have 3 injection events. The 2004.5+ is an awesome motor to get power from if you are going to just throw a tuner, exhaust, and CAI on it and be done with it, The 03-04 are not as responsive to tuners as the 04.5+ due to the 3rd injection event in the 04.5+ trucks, when ran with tuners they run hotter than any 03-04 which cause melting issues, also the 03-04 had weak valve seats and common injector probs, but the seats only drop when under High Heat. Which in an 03-04 getting it hot with the proper mods is not easy to do. Also the 03-04 get better mileage due to the fact that they only have 2 injection events rather than 3, such as 04.5+. The 03-04 have a mechanically controlled wastegate, whereas the 04.5+ have an electronic wastegate. Also like I stated before, the 03-04 are not as responsive on tuners as 04.5+, but when the 04.5+ reach 800-900hp they stall, there has not YET been an 04.5+ truck (325) that has reached 1000hp+ on #2 only. The 03-04s can and do, do it. Anything else just ask

So it's false info that the 04.5's had more horsepower than the 03-04's?
I did not say that, the 04.5+ do have a higher factory HP and TQ rating, I was stating that the 04.5+ is not the HO for the CR in general it is a different series. In a sense I guess you could say it IS the HO CR, but..since the motor belongs to a diff series of CRs I do not see it that way, forgive me if I confused you

No you didn't confuse me. I understand the difference between the versions. And I've seen Erick's truck firsthand, so I know what a 305 can be built to.

The point being, the OP wants to know what the difference is between an 04 and 04.5. And it's the factory power ratings - in addition to things you listed.
I covered most of them, if he has anything else he can ask, I will do my best to answer.

Never met eric, but I have read alot about the rig, it is nasty, I hope the 6.7 and 5.9 end up dominating the 6.4s and Lmms, I heard there are some 6.7s getting past 800hp, is this true? If so w/o spray they are ahead of the 6.4L, but in Febs edition of DPM there is an LMM GMC that has 1200hp:shock: Pisses me off, I am a Diehard Cummins fan, and hate to see the LMM or any truck surpass it (cummins) :(

Thought I heard something about valve seat dropping under high egt
in 03s an 04s yes, but until you get some $$$ it wont happen, you would basically have to try on purpose to drop a seat, dont get the 04.5+ jmo they run very HOT, and mileage isnt as good as 03-04, plus 04.5+s pistons melt alot easier, just my.02 either way it is a cummins
