what temp there i have buddy with 12 valve in cold weather it smokes until gets up to temp. Sounds like might be getting good fuel delievery if hard to start
No the head wouldn cause smoke. Ask him to take for a decent drive and see if it overheats. And ya it could have fuel line crack which like you said is lettin air in line
