Help identify this clutch disc

The clutch in my 99 was acting up... so I pulled it apart and found the disc had separated. I just spent 40 minutes looking at photos online trying identify who made it so I can ask about a warranty but I have found a match yet. It has a rather unique spring pack arrangement. Anyone recognize it?
