Is this truck new to you? If not how long has it made this noise? What clutch do you have? Have you checked trans fluid?

Also the transmission is the original with original syncros only thing he did to it was fix the 5th gear nut when it went out on him said that everything looked good inside tranny that there was no need to change out anything

Does it do it everytime you start? Warm and cold?

Start my disconnecting the turbo from the intercooler. Start the truck and see if the noise is gone. If it still persists then it's not junk bouncing around in the IC.

I would imagine it is in the tranny if the turbo disconnect doesn't do the trick. Could also be the starter.

Is the starter hanging up on the flywheel for some reason?

Maybe take a video of it? Iíll bet itís the starter, my truck always had a similar noise and when I replaced the starter it went away.
