Pulse at the start of 4th gear

I have a 1995 12v, 184k miles. I noticed a pulsing sensation at the start of 4th gear, especially noticable when towing. At first I thought it was my clutch going out. I had it replaced and nothing changed. It does not matter how quickly or slowly I let the clutch out, it still has that wobbling pulsing feeling. It will go away if I de-clutch, take it out of gear, and then put it back into 4th. Any Ideas on what could be happening?
