It’s very inconstant it might start doing it cold or hot in gear and in park it might not do it for awhile then do it a few times in a row

Ok, Miles on the truck and your fuel/power and timing setting? A monitor certainly would be helpful. Fuel filter is new?

See if you've got someone who has an advanced scan tool or MM or similar you can plug in to watch the rail pressure. I'm thinking three possibilities (kind of in order of likelihood, FCA and a popped relief valve are both about as likely, but it sounds more like an FCA) - FCA on the pump, popped relief valve, or restricted fuel filter. If it's the FCA you can have RP all over the shop or stuck high. If it's the relief valve, it'll be fine/normal, and then the thing unseats and it can't make pressure. restricted fuel filter usually won't clear up and she'll be gutless as all frig but idle fine.

Okay I have a plug so I’ll throw that in and check the voltage on the harness, fuel filter just replaced it’s been running fine and on off for months. With a spike in idle around 2 300 rpm then back down to normal

I have a similar problem:
The truck:
2012 Ram 2500
155K miles
Running RACE Me
EGR/DPF Delete
Glacier Diesel Big line kit
EDGE CTS Monitor

The problem
Cold Start, No smoke. No hard Start.
Rail Pressure (RP) runs steady 6 to 6.2K
When hot:
on Park RP is 4.8. 4.9K
on Drive, RP runs between 5.8 to 4.3K Yes a bit more thank 1k Fluctuation.
When fluctuation runs a bit fast thats when the DING comes on and P0191.
However, some times RP remains steady at around 5.3 to.5.7K
I've tried to see if the tank fuel level has something to with it, but it seems to do it at any level.
Have not felt power loss

So far I have replaced:
Fuel filter numerous times
Fuel rail and rail pressure sensor
and Fuel Control Actuator or MPROP

What I think I have left:
Fuel injectors
tank Fuel sending unit

Am I missing something? Any Ideas of what to do next?
