Ya, sounds like he has a stuck injector. That white smoke, does it act like steam sort of? I would make sure he gets the injectors checked or he could melt down a cyl. Happens a lot on the common rail trucks. Tell him to take it to someone who knows what they are doing. That for the most part would not be the dealer.

Just an FYI. The older technology, 12 valve and 24V with the VP44 injector pump, did not have a constant source of fuel under pressure at the injector all the time. So if an injector stuck open it would only get fuel when the pump said so for that cyl. The common rails have, at wide open throttle, between 23000 and 26000 psi of fuel "at" the injector so when they stick open they dump fuel all the time. This is why you melt pistons on the CR engines much easier than the older trucks. The shaking suggests it is running on at least 1 less cyl and lots of white smoke is classic unburned fuel. Please tell him not to drive it until he confirms it is or is not a stuck injector. It sometimes won't take long to do the damage.
P.S. I tried straight piping my 6.7 once and got a lot of white smoke on start up too. If the dealer can't figure it out. Have them try resetting the truck to the factory defaults. This made mine stop. Good luck. I will be curious to know if this cures it.
