What you describe is a different symptom from where you started in the thread and now sounds like the classic fuel starvation. Sounds like the lift pump is failing or can't keep up. The Juice on high setting is requiring more fuel. The cylinders do run cooler toward the front because of the location relative to the coolant inlet.

Me too. I missed that. So you have good FP when the Juice in on a higher level, pedal matted and the black smoke flying and misfiring. If so, I'd clear the 0216 code and see if it comes back. The MAP sensor could also still be in play.

Yep, I have good fuel pressure at all times. I need to get my hands on someone with a scanner to clear the codes for me. Tomorrow i'm going to replace the crank position sensor and the IAT. At ~$60 or so for the pair it's a good thing to do anyway. My starter loosened itself up and popped out of the bell housing about 3 days before the truck started running like this. I have a feeling the starter might have cracked the CPS when it fell off.

Still having troubles.... I'm officially out of ideas. I replaced the IAT and the CKS. No change besides curing the long start and sluggish tach issue. Anybody have any friggin clue what the hell is wrong?
