2014 5500 major engine failure

Last week a good friend of mines truck had a major engine failure. Truck is a 2014 Ram 5500 with 168K on it. Ran perfectly up until this point. We fully deleted and tuned this truck with PPEI "kory willis" about a year ago just running his smallest 30hp economy tune. Im very confused about what happened here and so are many other mechanics Ive talked to. Anybody have any ideas? Thanks!

#6 has a hole in it. Common.

That's some pretty funky looking spray pattern on the second pic.

Looks like injector failure to me. No, I'm not an engineer or anything.

What causes the hole in #6 most times? Yeah #5 has the small crack in the piston also. I shipped all 6 injectors out to Exergy for testing. Hopefully I can learn something from them!

I always thought the back cylinders got less air to them because of the stock intake and how it's towards the front of the block, which causes them to run hotter. Also it's the farthest away from fresh coolant, which means it runs hotter coolant wise. At least that's what a few cummins "gurus" have told me, I could be wrong, just going off what I've been told

2015 2500 Laramie ccsb CTD, Lots of thuren goodies, fox shox, bmf novakanes, amp steps, lost a few parts underneath, and the list goes on......

Could this happen if the truck was 100% stock? Everyone always told me having it deleted and a small tow tune should only help EGT's?

What were his egts running and RPMs when pulling hills etc...?

I'm a big advocate for having a Pyrometer. These trucks should come with them. I try to never let mine go over 1200F

A pyro will absolutely never prevent a melt down. If it's going to go, it's going to go. And you'll never know it by watching a gauge.....
