Any water jet shop will cut you anything you want. They usually charge a minimum order fee though. 175 seems to be about the norm. It's not very custom if you buy it from a store.

Depends on how creative you are. I built mine out of 1/4" Lexan, painted it graphite on the front, lazer etched the dodge R/T symbol to take the paint away from the front, painted the back bright red to shine through, kind of gives it a 3D look. With the new trans, I have an R/T switch under the dash for the different tunes, that's why I built that badge.
